First of all have a new account from Hotmail. While having new account, enter your personal information as in hacked one.
Then fill out below format in English and send it to passport@css.one.microsoft.com

- Your IP address. You may check it by going to http://www.whatismyip.com
- List of Secret Questions and Answers historically associated with your account
